Applicants Age 16 or Older (DS-11)
You should complete this form if you’re applying for the first time, or you don’t meet our requirements to renew your passport.
Applicants Age 15 or Under (DS-11)
You should complete this form if you’re applying for your child who is under age 16.
Applicants Age 15 or Under Without Both Parents Present
The DS-3053 is the Statement of Consent for Issuing a Passport to a Child and the DS-5525 is the Statement of Special Family Circumstances.
Adult Passport Renewal (DS-82)
Renew an existing passport issued to you when you were aged 16 or older.
Report and Replace Lost/Stolen Passport (DS-64 & DS-11)
You should complete this form if you lost your passport or it has been stolen. To replace a lost or stolen passport, you must apply in person and submit Form DS-11 in addition to Form DS-64.
Update Current Passport or Replace Emergency Passport
You may be eligible to use this form if you need to change or correct your passport, or you need to replace a passport that was limited to less than 10 years validity (which is the normal validity period for an adult’s passport).
You can fill out the correct form, or find information about your passport status at https://travel.state.gov.
You must provide one photo on a white background with your passport application. Our photo policies apply to adults and children. We have a few exceptions to our photo policies for infants.

Return by Mail to Malta and Gozo
If you wish to have your new passport or report of birth returned by mail, you must submit a self-addressed envelope of appropriate size and postage. Documents are sent via MaltaPost regular mail. The U.S. Embassy is not responsible for undelivered post. If you wish to collect your documents in person, you must collect all documents from the U.S. Embassy within 90 days of being notified of its arrival on dates and times sent you by email from the Consular Section.
Third Party Attendance at Passport Appointment
Generally, immediate family members may accompany passport or CRBA applicants to their appointment interviews at a U.S. embassy or consulate, and all minor children must be accompanied by a parent or guardian. Passport or CRBA applicants also have the option of being accompanied by an attorney at their appointment interview.
